If you have a concern with the service, actions, or lack of action by Threads of Life as an organization or a staff member or volunteer acting on behalf of Threads of Life, you may file your complaint verbally (by phone or in person) or in writing (by mail, fax, email) to the employee or volunteer with which you have your primary relationship with Threads of Life. All complaints received by Threads of Life staff will be handled according to the Complaints Policy and Procedure.

Whistleblowers

Threads of Life requires directors and employees to observe high standards of business and personal ethics in the conduct of their duties and responsibilities. Employees and stakeholders are encouraged to report financial or operational incidents or concerns by filling out the Whistleblower Report Form (PDF)* and submitting to Threads of Life using the instructions outlined on the form so that Threads of Life can address and correct inappropriate conduct and actions.

It is contrary to the values of Threads of Life for anyone to retaliate against any board member, employee or volunteer who in good faith reports an ethics violation, or a suspected violation of law, such as a complaint of discrimination, or suspected fraud, or suspected violation of any regulation governing the operations of Threads of Life.

*Fillable PDF form can be completed and saved using free Adobe Reader at get.adobe.com/reader or by printing and completing form by hand.
